What to expect

1. 1.Fill out the application. Enter the registrant's full legal name, date of birth, city of birth, and your relationship to the person named on the certificate.
2. 2.Complete a notarized sworn statement. California law requires a Sworn Statement verifying your authorized relationship to the certificate holder. Instructions for notarization are included with your application.
3. 3.Submit payment. The total cost is $45 per certified copy — a $31 state fee plus a $14 processing fee. See the full fees breakdown for details.
4. 4.Receive your certificate by mail. Once CDPH processes your application, the certified copy is mailed to the address you provide. Standard processing takes 7–10 weeks from the date CDPH receives your application.
